How to start up home health care in community?

I work at a private owned clinic and am interested in offering home health care for our surrounding community and our patients that are about to have surgery. Any information or suggestions where to look for information would be awesome. It just keeps getting more complex in the health care arena. I am a licensed physical therapist in California.
